- Contract or price for project
- Residential address for the last 2 years
- Employer’s name and address for the last 2 years
- W-2’s for the last 2 years
- Pay stubs (include year to date) for the last 30 days
- Verification of any additional source of income
- Bank, Credit Union and Investment statements for the last 3 months
- Personal Tax Returns for the last 3 years (if self employed or commissioned)
- Divorce decree if receiving or paying child support
- Social Security and Driver’s license numbers for all applicants
- Personal assets and liabilities information list
- Payment for appraisal
